Community of Hope is seeking a Community Health Worker to help promote appropriate use of healthcare services while improving access to care and empowering patients to make informed health decisions. As a Community Health Worker, you will serve as a vital link between healthcare providers and the community, fostering the improvement of health outcomes by providing health promotion education, care coordination and connection to resources to individuals and families. This is a full-time position located at our Family Health & Birth Center in NE, Washington, DC.
